The new 18,000 SF state-of-the-art outpatient clinic was planned and designed around a patient-centered medical home model, which is about providing assessment and care in teams. This team-based model of care, led by primary care physicians, provides continuous and coordinated care to ensure the highest level of healthcare.

The project includes 24 exam rooms organized into separate, flexible exam zones with each containing 12 exam rooms and procedure room centered around an open collaborative staff/physician work area allowing a direct line of sight to exam rooms and other visual cues for all team members.

Exam rooms are standardized in their design and contain the same supplies and equipment reducing confusion and inefficiency. After the exam, the patient is escorted to his or her next destination. On-site lab and radiology services are positioned between the reception/waiting and exam zones. A check-out desk is located at the only patient exit. This allows to engage with every patient exiting and ensure that all of their needs from the visit were addressed, including answering their questions and arranging follow-up visits, lab or imaging services or referrals. In addition, there is space shelled for a future 12 exam room zone, giving the Owner flexibility to expand as the market dictates.
